Child Plan (Age 13 & Under)
- 2 exams per year
- 2 professional cleanings
- Routine X-rays as needed
- Fluoride treatments
- 1 emergency exam per year
- 15% discount on additional treatment
Perio Maintenance Plan
- 3–4 periodontal maintenance cleanings per year
- 2 exams per year
- Routine X-rays as needed
- 1 emergency exam per year
- 15% discount on all additional treatments

Membership guidelines
Membership runs 12 months from the date of enrollment. Benefits cannot be used with dental insurance. Membership is non-refundable once services are used. Discounts apply only to treatment completed while the membership is active. Plans automatically renew annually unless canceled.

Which plan fits whom?
Adults join the Adult Plan, kids 13 and under the Child Plan, and patients in periodontal (gum) treatment the Perio Maintenance Plan. Call us and we'll match each family member in a minute.
How does the 15% discount work?
It applies to treatment completed while your membership is active and is taken right off your bill at the time of service. No claims, no paperwork, no waiting.
